Enrollment Form

A non-refundable enrollment fee of $50 per child is required at the time of enrollment. Credit or debit card information is required of all students as security, even if regular payments are by check.

For best results, please complete this online form, then bring or mail tuition and fees. It is safest to bring checks or card numbers in person, but if visiting the school would be impractical, please print, complete, sign, and mail the paper enrollment form (click here) to Gray School of Music, 11661 Preston Rd. Suite 136, Dallas, TX 75230. The paper form requires PDF reader software like Adobe Reader or Mac OS X's built-in Preview application.

Agreement (required):	I have read the school’s policy statement and understand that tuition is prepaid by the 10th of each month, and that there is a $20 late fee assessed if tuition is not paid by the 10th, and that a non-refundable enrollment fee of $50 per child is required at the time of enrollment. I understand that, in the event of late payment, my credit or debit card will be charged. I understand that a 2-week notice is required if a student is discontinuing lessons at the Gray School of Music, and that I am financially responsible for two weeks of tuition following drop notice.
Please submit one enrollment form per student online or in person, and bring or mail a $50 non-refundable registration fee made out to "Vicki Gray" for each child you wish to enroll in music lessons for the current term. If you are presently enrolled in the Gray School of Music, this registration will appear on your MAY INVOICE. Registration fees are due once a year and can be brought to the studio, or if paid by check, mailed to Gray School of Music, 11661 Preston Rd. Suite 136, Dallas, TX 75230. Please see the tuition page and policies page for more information about billing and recurring payments. If you have not already been scheduled for your first lesson with a teacher, someone will call you to schedule lessons as soon as this form is received and processed. All form fields are required, so if a field does not apply to you, please enter "NA".
